These terms apply to all clients submitting payment through the app. By completing payment, you acknowledge and agree to the following:


1. Service Scope

Anzara Health is a telemedicine-only, concierge endocrinology practice serving patients aged 18+ in CA, CT, FL, HI, NJ, NY, NC, and TX. (Anzara Health)

We offer a holistic, long-term approach to endocrine care through endocrinologists, nutritionists, and health coaches. (Anzara Health)


2. Payment & Plans

A‑la‑carte services (one initial consultation + one follow-up within 12 months; up to 90-day prescription, no refills): $1,100ⁱ (Anzara Health)

Membership Plans (12-month commitment starting with initial visit; includes three follow-up visits, monthly nutrition/coach evaluations, unscheduled doctor check-ins, direct communication): $240/monthⁱ (Anzara Health)

Payments are processed at scheduling/booking (a-la-carte) or start approximately three months after the initial visit (membership), charged to your card on file. (Anzara Health)


3. Insurance & Reimbursements

We do not bill insurance directly. Patients may receive a superbill to submit to insurers for out-of-network reimbursement, depending on their plan. (Anzara Health)

Payments may be eligible for FSA, HSA, or HRA reimbursement—verification with your plan is recommended. (Anzara Health)


4. Lab Work & Medication Costs

Testing is performed via LabCorp or Quest Diagnostics. Any related expenses not covered by insurance or not included in membership plans are your responsibility. (Anzara Health)


5. Communication & Emergency Policy

Providers respond to non-urgent inquiries within one business day, through email, phone, text, or portal. (Anzara Health)

The service does not handle emergencies. For urgent medical needs, contact your nearest emergency services or call 911. (Anzara Health)


6. Cancellation & Refunds

No refunds after payment is processed. (Anzara Health)

To prevent auto-renewal of membership, notify [email protected] at least 30 days before renewal. (Anzara Health)

If Anzara terminates membership, coverage continues through the end of the billing period without prorated refunds. (Anzara Health)


7. Privacy & Security

Your health data is protected under HIPAA. You have rights to access, correct, or request alternatives to how we communicate with you. (Anzara Health)

You may obtain a copy of your privacy notice or request restrictions on how data is used or shared. (Anzara Health)


8. Termination Rights

Anzara Health reserves the right to refuse or discontinue services for non-compliance, inappropriate behavior, or violation of these terms.


9. Acknowledgment & Consent

By completing payment through the app, you confirm that you have read, understood, and agree to this Agreement.
